State of MA Immunization Requirements Prior to Entrance to 7th Grade

School Immunization Requirements for the 2017-2018 School Year:

2 doses of varicella vaccine –chicken pox vaccine

1 dose Tdap vaccine-adolescent Pertussis, Tetanus, and Diphtheria

Please provide me with a copy of your child's most recent physical exam and immunization record.   Please bring, mail or fax  a copy of the information to the Lawrence School. Children who are not in compliance may be excluded from school until the school receives proper documentation of immunization. Ask your child’s doctor or visit the  link on immunizations for more information.
